• Nursing interventions carried out to meet the objectives (independent, dependent, and collaborative).
6. Evaluation
• Assessing outcomes against objectives.
• Modifying care where necessary.
7. Conclusion and Recommendations
• Summary of the care provided.
• Suggestions for continuity of care at discharge.
